  • How do I obtain additional or replacement identification cards?

    Call the Prominence Administrative Service Customer Service Department at 800-863-7515. Our team will ask for your social security number and will verify your home address. You can also send an email to [email protected], or you can access the member portal and request one at your convenience. It will take approximately two weeks for you to receive your new member ID card(s).

  • Where can I find information about my pharmacy prescription drug plan?

    Please refer to the front of your Prominence Administrative Services Member ID card for information about the pharmacy services available to you as part of your health plan.
