Care Management

The Prominence Care Management team is comprised of registered nurse reviewers and case managers with support from our Chief Medical Officer. The team focus is on providing access to appropriate, quality, cost-effective care for the employees and family members of each employer group.

Case management partners with those members who need this targeted care to ensure they get the care they need, when they need it, and thus serve as a valuable resource along the path to better health.

Employer-Based Wellness

The Prominence Health and Wellness program is designed for the entire member population with the intent of enriching physical, mental, emotional and occupational well-being. The goal is to make the work environment supportive of positive health behaviors and thereby help prevent chronic diseases and promote improved well-being for all employees.
